The existing 15"x 40'culvert will be removed and area restored <br /> with seed and mulch. New 15"x 40'culvert to be installed 190 feet to the west of old culvert location. All areas not stabilized <br /> with riprap will be restored with seed and mulch. Appropriate erosion control measures will be used to avoid sedimentation <br /> into Oak Lake during all culvert removal and installation activities. <br /> You have certified that your project meets the eligibility criteria and conditions for this activity. Based upon your signed <br /> certification you may proceed with your project. Please take this time to re-read the permit eligibility standards and <br /> conditions. The eligibility standards can be found on your application checklist or in the statewide general permit,WDNR- <br /> GP2-2017. The permit conditions are attached to this letter. You are responsible for meeting all general permit eligibility <br /> standards and permit conditions. This includes notifying the Department before starting the project and submitting <br /> photographs within one week of project completion. Please note your coverage is valid for 5 years from the date of the <br /> department's determination or until the activity is completed,whichever occurs first. <br /> Wetlands <br /> According to the project information submitted, 100 square feet of wetland fill will occur.This permit coverage constitutes the <br /> state of Wisconsin's wetland water quality certification under USCS s. 1341 (Clean Water Act s.401). <br /> Endangered Resources <br /> The Natural Heritage Inventory(NHI)was reviewed on June 4,2021. No known threatened or endangered species were <br /> found in or near the project limits. <br /> Waterway Best Management Practices <br /> The municipality is responsible for ensuring the project meets BMP standards before, during and after construction. A copy of <br /> water quality BMPs is attached to this letter. <br /> Waterway Recommendations <br /> This project is in a mapped flood plain area.The County Zoning office should be contacted with any questions about local <br /> zoning ordinances or NR116 requirements. <br /> Inspections <br /> The Department conducts routine inspections and may follow up to inspect your project to verify compliance with state statutes <br /> and codes. If you decide to modify your project, please contact your local Department Transportation Liaison to discuss any <br /> proposed modifications. <br />
