BURNETT COUNTY ZONING ADMINISTRATION <br /> BURNETT COUNTY GOVERNMENT CENTER <br /> 7410 COUNTY ROAD K,#102 <br /> SIREN,WISCONSIN 54872 PHONE: (715)349-2138 <br /> CONDITIONS OF PERMIT FOR CONSTRUCTION OF AN <br /> ACCESSORY BUILDING IN A FLOODPLAIN <br /> 1. The lowest floor or slab must be at or above elevation 932.8 <br /> *Use the RFE/BFE number for item 1* <br /> 2. A benchmark bearing the certification of a profession land surveyor or an engineer must <br /> be left within 100' of the lot/parcel and tied to NAVD88 datum. Documentation of the <br /> benchmark is to be submitted to the zoning office within 1 month of the land use permit <br /> issuance. <br /> 3. This structure cannot be used for human habitation per NR I I 6.12(2)(a)1. <br /> 4. This structure is firmly anchored to prevent it from floating away and restricting bridge <br /> openings or other constricted sections of the floodway. <br /> 5. All service facilities, such as electrical and heating equipments, shall be at or above the <br /> flood protection elevation. The flood protection elevation is 2' above the RFE* listed <br /> above per NR 116.12(2)(a)6. <br /> 6. Storage of any materials which are buoyant, flammable or explosive, or which in times of <br /> flooding could be injurious to property, water quality, or human, animal, plant, fish or <br /> aquatic life shall be either flood proofed to or placed at or above the flood protection <br /> elevation*. Adequate measures shall be taken to assure that these materials will not enter <br /> the floodway during an event per NR 116.13(6). <br /> 7. Compliance with the Endangered Species Act is required. Contact US Fish and Wildlife <br /> Service with any questions. <br /> I understand the above conditions for development in a floodplain. <br /> I understand and agree to have a certification completed and submitted by a registered <br /> professional engineer or professional land surveyor showing actual building and fill elevations <br /> within one year of permit issuance. <br /> I also understand that this certification will be used for the County's final certificate of <br /> compliance for this land use permit. <br /> Signature of Applicant Paul Garrity Date_7-14-017 <br />
