BURNETT COUNTY LAND SERVICES DEPARTMENT <br /> BURNETT COUNTY GOVERNMENT CENTER <br /> 7410 COUNTY ROAD K,#120O <br /> SIREN,WISCONSIN 54872 <br /> (715)349-2109 — <br /> IMPERVIOUS SURFACE MITIGATION AGREEMENT <br /> In order to secure a Land Use Permit relative to Burnett County L <br /> Shoreland Protection Ordinance,the following restrictions will apply <br /> to the use of any permitted structure permitted under WI Admin. <br /> CodeNR 115.05(1)(e)3.c.. <br /> 1. For lots that exceed 15%impervious surface area,the county <br /> shall issue a land-use permit that requires mitigation <br /> approved by the county and implemented by the property <br /> owner by the date specified in the permit. <br /> 2. Owners may choose option(s) from the mitigation table in <br /> the Burnett County Shoreland Code of Ordinance which Return to: Burnett County Land Services Dept. <br /> have been approved by the County Board and WI DNR. 7410 County Road K,#120 <br /> Owner agrees to implement the following options, as noted Siren,WI 54872 <br /> on the attached mitigation table: C <br /> 3. Owner agrees that failure to uphold the agreed upon provisions will result in the revocation of the Land Use Permit <br /> shown above which may result in citations and removal of the proposed structure. <br /> 4. The owner hereby agrees that at all times in the future, the real estate subject to this Agreement shall comply with all <br /> mitigation requirements of the Burnett County Shoreland Protection Code of Ordinances in effect at the time of signing <br /> of this document. <br /> S.
