AFFIDAVIT <br /> STATE OF WISCONSIN <br /> BURNETT COUNTY <br /> In the circuit court of the county of Burnett <br /> Jim Flanigan being duly sworn, says that on the 44 day of August, 2009, in said <br /> county, in and upon certain premises in the Town of Daniels of Burnett County and more <br /> particularly described as follows: <br /> Southeast Quarter of the Northwest Quarter (SE% NW%), Section Fifteen (15), <br /> Township Thirty-eight (38) North, Range Seventeen (17) West, Burnett County, <br /> Wisconsin. <br /> there now exists a necessity to determine if said premises comply with the Burnett County <br /> Sanitary Code 15.07(1) and appropriate requirements of the Wisconsin Administrative Code. <br /> The facts tending to establish the grounds for issuing a special inspection warrant are as <br /> follows: <br /> That the above said property is owned by Joseph Cycenas, et al who has constructed a <br /> building on the property, which appears to be a personal residence, or otherwise fit for human <br /> occupation. The property does not have any record nor has there been an application for a <br /> sanitary permit to install, or inspect an appropriate sanitary system. <br /> That under the Burnett County Land Use Ordinance, the Burnett County Sanitation <br /> Ordinance, and the Wisconsin Administrative Code any property built for human habitation is <br /> required to have a sanitary permit. <br /> That your affiant has contacted Edward Cycenas on two occasions and has received the <br /> following response; attached as Exhibit "A". <br /> Wherefore,Jim Flanigan prays that a special inspection warrant be issued to search such <br /> premises for said purpose. <br /> Subscribed and sworn to before me this m Flanigan_ <br /> �( day of August, 2009. <br /> � , . <br /> My Co�ission is (permanent Hon.
