a. Complete and Signed Water Resources Application for Project Permits (WRAPP) <br /> or Information Worksheet certifying project meets the terms and conditions of <br /> WDNR-GP2-2017. <br /> b. Project plans or schematic drawings <br /> c. Maps of the project site that show clear directions to the project site with project <br /> and property boundaries clearly labeled <br /> d. Photographs that represent existing site conditions where project will occur <br /> e. Alternatives Analysis for municipal highway, bridges, arches, and culverts, <br /> including detailed documentation that clearly demonstrates why the impact to the <br /> wetland cannot be avoided and how the impact to the wetland will be minimized. <br /> 2. You must apply to the department in writing not less than 30 days before <br /> commencing the activities authorized under this general permit. The notification <br /> shall provide information describing the activity, to allow the department to determine <br /> whether the activity is authorized by the general permit and shall give the department <br /> consent to enter and inspect the site. <br /> 3. For wetland discharges, the application must identify all activities affecting wetlands <br /> that will be conducted as part of a single and complete project, and shall include a <br /> detailed explanation of why the impact to the wetland cannot be avoided and how <br /> the impact to the wetland will be minimized to the greatest extent practicable. <br /> 4. Unless notified by the department to the contrary, the effective date of coverage <br /> under this permit is 30 calendar days after the designated DNR office receives a <br /> complete application package. <br /> 5. The department may request that you provide additional information necessary to <br /> verify compliance with the terms and conditions of this permit. The department may <br /> make a request for additional information one time during the 30-day period. If the <br /> department makes a request for additional information, the 30-day period is paused <br /> on the date the person applying for authorization receives the request for additional <br /> information. The clock remains paused until the date that the Department receives <br /> the information at the designated DNR office, at which point the clock resumes from <br /> the point it was initially stopped. <br /> 6. If adverse weather conditions prevent the department from conducting an accurate <br /> on-site inspection or assessing the potential for site-specific impacts during the 30- <br /> day application period, the department shall give notice to the applicant of this <br /> condition and shall complete the inspection as soon as weather conditions permit. <br /> *Note* Wetland inspections are typically completed in the growing season. Please <br /> coordinate with the DNR Transportation Liaison regarding growing season in your area. <br /> 7. The Department may determine the project is not eligible for this general permit and <br /> require that the project be reviewed through the individual permit process outlined in <br /> Wis.
