Please note, additional permitting information can be found by following the <br /> hyperlinks throughout the electronic version of this document. <br /> 44 ` Project Description <br /> 0'i Site 1 is Frog Lake Road over tributary to Namekagon River. Existing 36" x 54' culvert <br /> 7/'6 washed out after a beaver dam upstream failed. Was replaced with a 48" x 58' culvert. (SE 1/4 SW <br /> 1/4 Sec. 13, T41 N, R14W). <br /> ��St •Site <br /> 4, 2 is North Bear Lake Road wetland culvert. Existing 36" x 48' culvert replaced with a 36" x <br /> /a oe 52' culvert (NW 1/4 NW '/4 Sec. 20, T41 N, R14W). <br /> Z <br /> ,7,(17- 1You have certified that your project meets the eligibility criteria and conditions for this activity. <br /> ,L(qoBased upon your signed certification you may proceed with your project. The eligibility standards can be found <br /> on your application checklist or in the statewide general permit, WDNR-GP2-2017. The permit <br /> conditions are attached to this letter. You are responsible for meeting all general permit eligibility <br /> standards and permit conditions. This includes notifying the Department before starting the project, <br /> and submitting photographs within one week of project completion. Please note your coverage is <br /> valid for 5 years from the date of the department's determination or until the activity is completed, <br /> whichever occurs first. <br /> Wetlands <br /> According to the project information submitted, 200 square feet of wetland fill will occur. This permit <br /> coverage constitutes the state of Wisconsin's wetland water quality certification under USCS s. <br /> 1341 (Clean Water Act s. 401). <br /> Endangered Resources <br /> The Natural Heritage Inventory(NHI)*was reviewed on June 8, 2020.
