b. An additional permit fee is not required to place fish stick structures at additional locations under an existing general permit coverage where the initial applicant was granted permit coverage for additional locations. A person seeking to place additional structures under an existing permit coverage must provide to the Department a request containing all the following: i. The docket or identification number of the initial general permit coverage to which you seek to add new locations. ii. First and last name of applicant seeking coverage of new locations. iii. Applicant's contact information, including but not limited to,street address, city, state, zip code, telephone, and e-mail. iv. A copy of the deed or similar proof of ownership of the site where the activity will occur. If you do not own the site, also include proof of any notice(s) and permission(s) required by Section IA, Standards 2 and 3 of this Genera Permit. v. Project diagram that shows all the following: 1. The location of the project relative to the OHWM and the bank toe. 2. The location of silt fences or any other sediment control devices. 3. A description of materials and equipment to be used for the placement of the fish habitat structure, including the location of the live trees to be used and the purpose and need forthe placement. vi. Maps of the project site that include all the following: 1. Property lines for the property where the project will occur including identification of the riparian zone area from the OHWM to the line of navigation. 2. The location of the proposed fish habitat structure. vii. Photographs that represent existing site conditions where the project will occur. viii. Signature. The signature on any cover letter submitted for the purpose of applying to be covered under existing general permit coverage will certify that the applicant meets the requirements under Section 3 of this general permit. c. Permit coverage for additional structure locations terminates 5 years from the issuance date granting coverage for the additional structure locations. SELF CERTIFICATION CONDITIONS 1. You must notify Nathan Walker at (715)492-0243 or nathan.walker@wisconsin.gov before starting construction and again within 5 days after the project is complete. 2. You must complete the project as described on or before 12/30/2025. If you will not complete the project by this date, there is no opportunity for an extension and you must apply for a new permit. 3. Your signed certification does not authorize any work other than what you specifically describe in your application and plans, and as modified by the conditions of your signed certification. If you wish to alter the project, you must first obtain written approval of the Department.
