b. An additional permit fee is not required to place fish stick structures at <br /> additional locations under an existing general permit coverage where the <br /> initial applicant was granted permit coverage for additional locations. A <br /> person seeking to place additional structures under an existing permit <br /> coverage must provide to the Department a request containing all the <br /> following: <br /> i. The docket or identification number of the initial general permit <br /> coverage to which you seek to add new locations. <br /> ii. First and last name of applicant seeking coverage of new <br /> locations. <br /> iii. Applicant's contact information, including but not limited to,street <br /> address, city, state, zip code, telephone, and e-mail. <br /> iv. A copy of the deed or similar proof of ownership of the site where <br /> the activity will occur. If you do not own the site, also include <br /> proof of any notice(s) and permission(s) required by Section IA, <br /> Standards 2 and 3 of this Genera Permit. <br /> v. Project diagram that shows all the following: <br /> 1. The location of the project relative to the OHWM and the bank toe. <br /> 2. The location of silt fences or any other sediment control devices. <br /> 3. A description of materials and equipment to be used for the placement of the <br /> fish habitat structure, including the location of the live trees to be used and the <br /> purpose and need forthe placement. <br /> vi. Maps of the project site that include all the following: <br /> 1. Property lines for the property where the project will occur including <br /> identification of the riparian zone area from the OHWM to the line of navigation. <br /> 2. The location of the proposed fish habitat structure. <br /> vii. Photographs that represent existing site conditions where the <br /> project will occur. <br /> viii. Signature. The signature on any cover letter submitted for the <br /> purpose of applying to be covered under existing general permit <br /> coverage will certify that the applicant meets the requirements <br /> under Section 3 of this general permit. <br /> c. Permit coverage for additional structure locations terminates 5 years <br /> from the issuance date granting coverage for the additional structure <br /> locations. <br /> SELF CERTIFICATION CONDITIONS <br /> 1. You must notify Nathan Walker at (715)492-0243 or nathan.walker@wisconsin.gov <br /> before starting construction and again within 5 days after the project is complete. <br /> 2. You must complete the project as described on or before 12/30/2025. If you will not <br /> complete the project by this date, there is no opportunity for an extension and you <br /> must apply for a new permit. <br /> 3. Your signed certification does not authorize any work other than what you specifically <br /> describe in your application and plans, and as modified by the conditions of your <br /> signed certification. If you wish to alter the project, you must first obtain written <br /> approval of the Department. <br />
