Box 22 <br /> Siren, WI 54872 <br /> Dear Mr. Cycenas: <br /> As required by the Burnett County Private Sewage System Ordinance, you are hereby <br /> advised that you may be in violation of Section 15.14(3) and Wisconsin State Statute <br /> 145.135(1) and Section 15.08 Incorporating Wisconsin State Statue 145.06(1)(a). <br /> The following violation(s) have been noted: <br /> INSTALLING A PRIVATE ONSITE WASTE TREATMENT SYSTEM <br /> WITHOUT A SANITARY PERMIT AND INSTALLING A PRIVATE ONSITE <br /> WASTE TREATMENT SYSTEM BY AN UNLICENSED INDIVIDUAL. <br /> The first violation and/or reporting of such are noted as having occurred on August 5, <br /> 2009.
