• <br /> To abandon and remove the dam an application will need to be submitted that includes the following items.The link for <br /> the application can be found on this webpage: https://dnr.wisconsin.gov/topic/dams/DamRemovalAbandonment.html <br /> 1. Application form <br /> 2. Application fee (with after-the-fact fee addition) <br /> 3. Ownership documentation <br /> 4. Plans and specs <br /> • Existing and proposed profile of stream in 25-ft increments 250-ft up and downstream.This should <br /> include a labeled point where the sheet pile is and what it will be removed to. <br /> • Existing and proposed cross sections two upstream and two downstream with one cross section at the <br /> sheet pile location.The most upstream and downstream cross sections should be located where work <br /> will begin and end.The other two cross sections should be between the sheet pile and the most <br /> upstream and downstream cross section. <br /> • Slopes should be in a stable configuration (no steeper than 2:1—horizontal:vertical). Outline material <br /> proposed to stabilize the channel and side slopes. <br /> 5. Narrative description: <br /> • What the project is, purpose of project, and need for the project. <br /> • Information on other participants if this is a cooperative effort. <br /> • The parts of the dam that will be removed to render it abandoned. <br /> • How you intend to carry out the project, including methods of dam removal, materials, and equipment. <br /> • Your proposed construction schedule and sequence of work. <br /> • The location of any disposal area for the dam materials. <br /> • The stream channel and flowage bed restoration including protection needs. <br /> 6. Riparian owners list- List the names and addresses of the adjacent property owners. <br /> 7.
