BURNETT COUNTY ZONING ADMINISTRATION <br /> BURNETT COUNTY GOVERNMENT CENTER <br /> 7410 COUNTY ROAD K,#102 PHONE(715)349-2138 <br /> SIREN,WISCONSIN 54872 <br /> December 13,2006 <br /> Nils Koerper <br /> 12396 County Road D <br /> Grantsburg,WI 54840 <br /> RE: Repair of Mound type POWTS for Mike Moritz <br /> Dear Nils, <br /> As per our on-site inspection and evaluation of the performance problems with the mound type Private On- <br /> site Wastewater Treatment System at the residence of Mike and Kelly Moritz at 12431 North Fork Drive, <br /> this letter is an Authorization to make the necessary repairs described below. We concluded that thel/8 <br /> inch diameter holes in the distribution laterals are plugged. We also noted that the pump has failed. The <br /> proposed repair is to excavate to the laterals by hand,carefully removing and saving the aggregate,to be <br /> replaced later,and removing,cleaning,and reinstalling the laterals,after redrilling the holes to no larger <br /> than 3/16 diameter.The pump will be replaced with a Zoeller 152. The initial system installation required <br /> 15.58 gallons per minute at 19.1 feet of head. The larger 3/16 inch diameter holes will demand 24.9 <br /> gallons per minute at 16.06 feet of head. The Zoeller 152 will supply close to 50 gallons per minute at 16 <br /> feet of head. Because no significant change will be made to the POWTS,no plan revision,or Sanitary <br /> Permit,will be required. I will inspect the repair of the POWTS on Friday,December 15,as we have <br /> already scheduled. Should we discover additional problems beyond the plugged holes in the laterals,which <br /> we believe to be the problem, a Sanitary Permit will be required for a more comprehensive repair of the soil <br /> absorption cell. <br /> Upon completion of the repair,I will file an inspection report with Sanitary Permit Number 445614(Co.# <br /> 28158),outlining what we observed and actions taken to repair the system. <br /> Sin y, <br /> Craig�onroy <br /> Assistant Zoning Administrator <br />
