Please note, additional permitting information can be found by following the <br />hyperlinks throughout the electronic version of this document. <br />Proiect Description <br />Installing three wetland culverts along Fish Lake Rd. at Fish Lake. Two are existing structures <br />and another culvert will be installed at a new location. <br />You have certified that your project meets the eligibility criteria and conditions for this activity. <br />Based upon your signed certification you may proceed with your project. Please take this time to <br />re -read the permit eligibility standards and conditions. The eligibility standards can be found <br />on your application checklist or in the statewide general permit, WDNR-GP2-2017. The permit <br />conditions are attached to this letter. You are responsible for meeting all general permit eligibility <br />standards and permit conditions. This includes notifying the Department before starting the project, <br />and submitting photographs within one week of project completion. Please note your coverage is <br />valid for 5 years from the date of the department's determination or until the activity is completed, <br />whichever occurs first. <br />Wetlands <br />According to the project information submitted, 300 square feet of wetland fill will occur. This permit <br />coverage constitutes the state of Wisconsin's wetland water quality certification under USCS s. <br />1341 (Clean Water Acts. 401). <br />Endangered Resources <br />The Natural Heritage Inventory (NHI)* was reviewed on May 18, 2018 While it is unlikely the <br />proposed project will impact the following resources that have been observed in the project area or <br />its vicinity, please be aware of their potential presence. <br />Robbins' Spike-rush (Eleocharis robbinsil), a Wisconsin Special Concern plant, is found forming <br />beds in the shallows of soft -water ponds and lakes; these areas are characteristic of lagoons on <br />the Apostle Islands.
