BURNETT COUNTY LAND SERVICES DEPARTMENT <br /> BURNETT COUNTY GOVERNMENT CENTER C <br /> 7410 County Road K,#120 Phone(715)349-2109 <br /> Siren,Wisconsin 54872 landservices@burnettcounty.org C <br /> r <br /> NOTICE OF ACTION ON CONDITIONAL USE PERMIT <br /> b <br /> June 15, 2020 <br /> Wood Lake Camp &Retreat, LLC <br /> 14309 Lake Drive NE <br /> Columbus, MN 55025 <br /> RE: CONDITIONAL USE PERMIT #CUP-20-18 O0 <br /> C <br /> Dear Property Owner, et <br /> At the June 3rd, 2020 meeting of the Land Use and Information Committee, your <br /> application for a Conditional Use Permit application was considered. <br /> On the basis of the evidence presented at the hearing on this application, it was <br /> determined that a Conditional Use Permit be GRANTED subject to the following <br /> conditions: <br /> 1- A facilities map needs to be completed for the entire property and filed with the <br /> eD <br /> zoning office (signed by a licensed architect, engineer or surveyor)which shows <br /> the location of all roads, property lines, setbacks, structures, water supplies, <br /> private waste disposal system(s), and any other information the Land Use and <br /> Information Committee shall deem necessary by November 30, 2021. Any <br /> proposed changes in the approved facility shall be presented to the zoning office <br /> for approval. No implementation of the proposed changes shall take place until <br /> written approval is received from the zoning office. <br /> 2- Sanitary system(s) shall be installed as required per county and state code. <br /> 3- All sanitary systems to be serviced/maintained on the required interval. <br /> 4- Land Use permits to be obtained for all items before construction/relocation. <br /> 5- Quiet hours of 11 PM to 7 AM shall be maintained by the owner/operator for all N <br /> thJ <br /> activities taking place on the parcels Friday and Saturday. Quiet hours of 10 PM <br /> to 7 AM shall be maintained by the owner/operator for all activities taking place N <br /> on the parcels Sunday through Thursday. N <br /> 6- Follow all local, county, state and federal requirements for this activity. <br /> 7- A stormwater management plan shall be provided to the County by 11/1/2020, as O0 <br /> proposed by the applicant. <br /> Page 1 of 2 <br />
